Medical transport

I provide a Non-Emergency Medical Transport service from Le Havre and the entire surrounding area, dedicated to patients who need reliable, punctual and comfortable assistance to reach their medical appointments.

This service is suitable for:

• medical consultations

• hospital admissions and discharge

• specialised appointments

• dialysis, radiotherapy, chemotherapy sessions

• examinations (MRI, CT scan, X-ray, blood tests, etc.)

• appointments in medical centres, clinics and hospitals

The service is carried out in a clean, comfortable and well-maintained vehicle, with a professional, serious and attentive driver.

My goal is to make your journey simple, safe and stress-free, whatever your situation.

I can assist you with:

• getting in and out of the vehicle

• accessing the entrance hall or the reception desk

• carrying your medical documents

• waiting on site if needed (optional)

• providing the return trip after your appointment

This service is intended for autonomous patients who do not require a stretcher or a medical wheelchair.

To ensure that your medical transport is covered and that the process runs smoothly, it is essential to follow the rules below regarding Medical Transport Prescriptions (PMT).

1. Your doctor issues a “Medical Transport Prescription”

This document may take several forms:

• Medical Transport Prescription (PMT)

• Standard Transport Form

• Non-Emergency Medical Transport Order

Without this document, the health insurance fund cannot cover the transport.

2. Presenting the document to the driver

The document must include:

• full name

• reason for the transport

• location of the appointment

• date and time

• doctor’s signature

✔️ The appropriate boxes are ticked:

▪ ALD (Long-Term Illness)

▪ Workplace accident

▪ Series transport

▪ Hospital admission

▪ Hospital discharge

▪ Etc.

I check that all information is correct before beginning the trip.

For your health insurance to accept it, the Medical Transport Prescription (PMT) must be:

▪ accurate

▪ complete

▪ signed by a doctor

▪ consistent with the reason for the transport

Trips over 150 km: Prior Agreement Required

For any journey exceeding 150 km, a specific procedure applies:

you must obtain a Prior Agreement Request.

This document is similar to the PMT but consists of three pages.

Procedure:

1. Send the first two pages to your health insurance fund at least 2 weeks before your appointment

2. Keep the third page and give it to the taxi

3. If the insurance fund does not reply within 2 weeks → the agreement is automatically approved

In case of a medical emergency

If the delay before your appointment is less than 2 weeks:

▪ contact me immediately

▪ the doctor must tick the box “Transport for urgent medical reason” on the Prior Agreement Request

3. Outbound and return transport

I accompany you:

• from your home

• to your medical centre, clinic or hospital

• and back to your address after the appointment

4. Your responsibilities (and not the taxi’s)

Contrary to common belief, the taxi is not responsible for your medical documents.

You are responsible for:

✔️ Requesting your documents from your doctor:

• PMT (Medical Transport Prescription)

• Prior Agreement Request

• Certificate of attendance (if required)

✔️ Checking that all information is accurate

✔️ Sending your Prior Agreement Request to your health insurance fund within the required deadlines

The taxi cannot complete or modify these documents.

5. Transmission of documents to the health insurance fund

Once the transport is completed, I handle the necessary administrative procedures with your health insurance fund.

I cover all medical facilities in the region, including:
